Cataract [in Latin cataracta – water fall] is the opacity of the lens of the eye, its capsule or both. It can be of different types - capsular, lamellar, nuclear, cortical, morgagnian, congenital, infantile, traumatic, diabetic and senile. An estimated 1.25 million people are blinded by world wide cataract each year. See: Picture. The stages of cataract are as follows: the incipient stage (spoke shaped opacities, cloud like opacities, opacity of cortex or nucleus), the swelling or immature stage (swollen lens, shallow anterior chamber), the mature stage in which the lens has become completely opaque and the hypermature stage (lens becoming either solid and shrunken or soft and liquid) Symptoms: At first the vision becomes distorted, particularly during the night driving or in very bright light, due to light sensitivity (photophobia). As the cataract progresses, sever visual impairment develops. Etiology: The common form is a result of aging. Other forms may be congenital or caused by infection, injury, exposure to radiation or adrenocortical hormones taken for long periods. Cataracts may also occur as a complication of diabetes. Premature cataract development has been linked to cigarette smoking. Orthodox Surgical Treatment: Surgical removal of the lens is required unless associated inflammation is present. In the past, this procedure required an incision large enough to permit extraction of lens and implantation of the lens. Now the procedure can be done through very small 3mm incision in the cornea. Ultrasound is used to fragment the cataract through the incision. The replacement lens is folded and inserted through the small incision.
